PUERTO BOLIVAR, ECUADOR, APRIL 20 - 22, 2007  
 

Charity Anywhere Foundation EC, under the direction of Dr. Washington Zambrano. And the Ecuadorian Navy, under the direction of CPNV-EMC Oswaldo Zambrano took off Friday, 20th  at 9:00 am from the CAF EC headquarters (Abascal street). This new service mission was meant to start at the province of El Oro, city of Bolivar’s Pourt (this destination was 12hrs. away from Quito).
The person encharged of the medical coordination was Dr. Olga Farinango, the dental coordinator was this time Dr. Isabel Zambrano, and the coordinator of construction was Eng Julio Zambrano. CAF’s volunteers were Dr. Arias, Alvear, Tapia, as dentists. Dr. Ortega, Moya, General Physicians and RN Cármen Lascáno. Pharmacy was under the administration of Mss Marianela Ruiz and Juan F. Zambrano 
This project started at 9:00 am and it went until 4:30 pm. On the Medical field, the following specialties were offered: Gynecology, General Surgery, Pediatrics, Ophthalmology, General Intern Medicine.
ON GENERAL MEDICINE were attended 158 patients not being able to find too much variation as far as those pathologies found on previous service missions. Out of all the consults we can conclude that an 18.4% of all the patients saw our doctors because of articulating chronic pain, Mialgies; followed by a 17% consults due to low-back aches. A 12% of our consults were due to Gastric Problems and Dispesis, followed by Parasitoids with a 10, 7 % which is fairly high too.
On this group of patients, where old age added to bad eating habits have more incidence medical problems such as High Blood Circulation problem (Hypertension), Dislipemies with a 7,8 %. The High Respiratory-Track illnesses like Pharynx, Bronchitis Inflammations, Allergy Sinus, and night-time Cold were diagnosted on a high 13, 4% rate out of all the patients. There also were Urinary Infections on a 6, 4%. We may conclude that once again we have seen the same Pathologies that are always found in each service mission; with a small difference that depend upon the whether, food and the geography of the Region.
ON PEDIATRICS we can say that 192 patients were seen, and with doubts about it, Intestinal Parasitoids is the prevalent illness with 39%, followed of High Respiratory Infections with a 26% which coincidently enough happen during the Rainy Season on this particular region of Ecuador. Coughing with a 6% and Bronchi Hyperactivity with a 5% can also rise up their percentages owed too to the Rainy Season. By being an already financially depressed region, young children on its majority tempt to be underweighted. What it really was notorious was that the Chronic Diarrhea Illness (EDA) is not longer found with the same incidence as it used to be a couple years ago. Another possibility is that parents that know EDA is a medical emergency could have taken their children to the nearest hospital on their region. In any case, only a 2, 3% of our patients had this pathology. On the Pediatric Group, only 5% out of our hole number of patients were found as healthy children; and the Dermatologic Problems (Dermatitis, Piodermis, Skin Mycosis) were found on a 9,3%. Because of the Region where we worked, Classic Dengue was found as the diagnose for three cases , which tell us that probably existed some previous cases that didn’t consult us or simply that were diagnosed as another pathology.
On the SURGICAL AREA, we couldn’t do any surgical procedures owed to the lack of facilities the Ecuadorian Navy didn’t have ready for us to do some Surgery. We should mention that an Appendicitis was detected (that was sent to the Hospital), and a Preputial Quits that was fixed immediately.
On the GINECOLOGY AREA were 14 consults of which 50% of them needed to see our doctors thanks to I.V.U. and a 42% because Candidacy Vagina. The EPI was presented on a case. Four pregnant women assisted to their monthly control, and two patients got a consult because of a Dysfunctional Hemorrhagic disease. With a total of 366 patients, medicine was distributed by the Pharmacy on a total of 453 people.
On DENTISTRY was seen 214 patients getting done 45 extractions, 40 cleanings, 41 fillings, 108 fluoride treatments, which makes up a total of 234 dental procedures. It should be mention that 75 dental kits were given up.
The wonderful experience of sharing with needed people that arrive to us to receive our help finding a way to get better from their medical problems continue to be the main reason for us to keep doing a good work “anywhere and for anyone”.
It should be mention that the Ecuadorian Navy also distributed medicines, glasses and toothpastes. After this journey of work, the volunteers of CAF came back to Quito on the morning a Sunday, 22nd of April 2007.

 
CUTUGLAGUA - ECUADOR, MAY 12, 2007
 

Having a new expectation of doing our labor this time in association to  the “Mariana de Jesus” foundation, once again, some volunteers-with the desire to serve our follow friend, followed Dr. Washington Zambrano, (general director, and Julio Zambrano general coordinator) to do a service mission at the populous ward of Cutuglagua. This area is very poor and its population needs a lot of medical assistance in any way. Some of the known needs this neighbohood has are basic services such as a good suit system, good roads, running water, public electricity, clinics, phone lines, etc. The absence of running water in Cutuglahua forces to its inhabitants to get the vital liquid from delivery trucks. These trucks sell water on very expensive prices, which makes even harder the living conditions for the locals. The sanitary conditions were terrible. Therefore, the Epidemiological profile was very predictable and noticeable to us. Although we were at the end of Quito South, at some point we almost believed we were on a rural area or in a small village far away from the capital city of Ecuador. We went there preparing three video conferences, but unfortunatly we were only able to show one that talked about Oral Hygine for Children. What we were hoping to show was a conference about Aids along with another on Alcoholism to young folks as well as adults that lived there in Cutuglahua. Unfortunatly, very few locals showed up, which forced us to quit on showing the two video conferences explained above.
Along to Dr. Alberto Vargas (Medical Coordinator), another doctors also helped us on this time. They were German Polo, Jose Molina, Gloria Polo, Fabricio Polo, and  nurse. Additionatly to them, even our Attourney of Law, Oswaldo Veloz worked registrating our patients. Seventy two patients were children and seen by the Pediatritian. A 37,5 % of them had High Respiratory deseases like three case of Bronchitis and Neuomonia, Infants Cold (which includes stuffy nose, fever and pain) Runny Nose, Coughing, Commun Flu, Sore Throat, Itchy Eyes, and some other allergy symptoms.   Weather conditions plus natural high elevation of  Cutuglagua and the assolation this neighborhood has affected the people that live there on many ways. A 66% of the pediatric consults were Intestine reletead problems, specially Parasites and chronic Diarrea. Although it was true that we could not find children suffering of a chronic dihydration, Diarrea by itself had a total 13,8% out of our total of pediatric consults, which still is very high. The  Kwashorkor Malnutrition was found there in Cutuglagua, and also some cases of Low Weight on a total of 13,8% out of our total of pediatric consults. Various open markets funtion near by the area geographical of Cutuglagua, therefore we can assume that was the reason to think malnutition problems were relatively low. At the other hand, an 18% of our pediatric patients had skin releated problems, specially Micosis. Many  families, for not saying almost all of them, had several children at the time.             Although too many children lived at the same house, only two cases of Escabiosis (Sarcopty Scobie) were found and only on a child there. Another illnesses  wich includes visual limitations were also found. Conjuntivis and some other eye-problems had low relevance with a 7%. Only three children had Great Mal. Within the adult population, 110 people were seen, of which 22,7% had Parasites. That confirms our hypothesis that relates bad living conditions, bad eating and hygine habits with the health conditions on the population. A 17,2% of our consults were out of bone related problems, which also includes low back aches, and articulating problems that were owed living conditions and the cold weather. Four patients were diagnosted with Reumatic Arthitis.  Headaches with unknown origen were found on 12,7% and with a similar percenatge headache/cold related problems. The medical illnesses related with the Superior Respiratory Duct were present on a 13,2% out of all of our consults, specially Pharingitis and Rynopharingitis.

Anemia was present with a 11,8% along with blood issues, particularly the hematopoyetic issue of Poliglobulia on a 9,6%. Although these diagnosts were based upon the related symptoms told by our patients and not by the confirmation of our Clinical Laboratories. A 10% of our patients had diferent skin problems, specially those like Dermatomicosis. And with a similar percentage, we found Gastrointestinal problems, specially Dispesias and Gastritis. High Blood pressure was low, which is completely diferent of what we have found on people that live on sea-level like coast, jungle or islands. Only a 5% of the Eldery People had this medical limitation (Hypertension). Obviously, each patient of us was able to get individually their prescription from the pharmacy we stablished there too. We gave up 192 prescriptions.
On the Dental Department we had Drs. Isabel Zambrano, Ximena Tapia, Carla Arias, Monica Zambrano, Said Mangui (Maxilofacial Surgeon). We also counted with Jaime Castro, Nataly Mier, Gloria Pilatasig and Juan Zambrano. A total of 113 patients were taken care on which 200 dental procedures were done. The results of our procedures tell us that 55 Exodontial Jobs were done, 55 Restorative Fillings were done, 46 Ultrasonic Cleanings were done, and 44 Flouride treatments were given too. Somewhere between 4 pm. we finished our mission knowing help was given to those in real need, not very far from down town Quito.

 

PILLARO - ECUADOR, JUNE 1 - 2, 2007
 

Pillaro is ubicated at the north part from the Tungurahua province, and has a surface of 443,1 square Kms. Its stimated population is about 37,224 inhabitants. They all are humble and most of them are dedicated to do farming. On a bus ride we reached Pillaro after traveling for aproximately 3 hours South from Quito. On a Friday, June 1st, 2007 we left our headqurters with the following group of volunteers.- Dr Ramirez (Plastic Surgeon), Dr Jenny Naranjo (Stethic Practitioner), Dr Gloria Polo (General Practitioner), Dr. Alberto Vargas (Epidemiology), Dr. Jose Molina (General Practitioner), Dr. Isabel Zambrano (Pediatric Dentist), Dr Ximena Tapia (General Dentist), Mss. Alvear, Mr Carvajal and Mr Cortez (Dental Students), Dental Lab Technician Carlos Tipanluisa, Economist Maria Guzman (General Coordinator for Projects), Juan Zambrano (Pharmacy), Carlos Celin (Logistics) and also Josue and Mathias Zambrano as our youngest volunteers. Thanks to the colaboration of Dr Edwin Cortez (Pillaro’s major), Dr. Dora Rivadeneira (who is the President of the City’s Wellfare Department), Dr. Vela, Dr. Carrillo, Dr. Marinio, Dr Haro, and the Nurse Practitioners , Viera, Burgos and Toapanta.
As always… with a the desire to serve our mostly needed fellow friends in Ecuador, and with the idea of adding a bet of relief for the welfare of some, we arrived to our destination where after setting up our clinics and equipment, we formally started our Service Mission. Eventhough the school year had already finished there in Pillaro, we had an attendance of about 150 young fellows to our “Human Sexuality” presentation performed by Dr. Alberto Vargas. We were absolutly very satisfied by the great attendance to this Presentation that hopefully will help understand on the Pillaro’s Young Population the methods to promote a Healthy Sexuality and the prevention of related illnesses.
We were able to came up with 306 medical consults, of which 15 were exclusivly Plastic Surgury. On Dermatology (Skyn Care Science) we did 28 consults and the left over consults were General Medicine as well as Internal Medice. A total of 305 men came, which is a 34,3 % out of our total of consults. A total of 201 women came, which represented a 65,7% of our total consults. As far as an Age Range Distribution, we had children younger than a year on a total of 8 individuals (which is a 2,6% of our consults). Children from 1 up to 5 were 29 individuals (which is a 9,5% of our consults), from the age of 6 up to 15 we had 25 patients (which is an 8,1% of our consults), from the age of 16 up to 45 we had 95 patients, (which is a 31,1% of our consults), from the age of 46 up to 60 we had 19,6 patients, (which is a 19,6% of our consults), from the age of 60 and beyond we had 89 patients, (which is a 29,1 % of our consults).
Contrary of what is normally seen in another towns, it really was interesting to see the large amount of Eldery people been attended by us during our days of attention. It should be also mentioned that we also had the time to visit the Local Eldery Home where we donated 40 medical prescriptions. Therefore, it was notorious to see how easily was to find people beyond 60 and how difficult was to find babies under the first year of live. A very curious situation was also the absence of pregnant women at the medical consults. Eventhough, this observation doesn’t agree with the Population Pyramid provided by the goverment back in 2001, we were able to observate a base that tents to be very small which doesn’t match with what was seen over our two days of Consults and Service there in Pillaro.
As far as what was found for Pathologies we observed that on under a year babies the Parasite Illness was fairly often followed by the illnesess of the Respiratory Track and with a low percentage (7,5% Mal- Nutrition). Between the group from 1 up 5 year olds a 44,8% did consult with us because of Intestinal Poli-Parasitosis, whereas the 13,2% because od Mal-Nutrition, although it wasn’t of the Marasmatic kind   but Kwashorkor one. The illnesses from the Respiratory Track occupied an important place with a 33% of our total number of consults. It was observed two cases of enheritaged Hart Disease, to whom it would be great to do some kind of feedback to be able to solve this medical limitation for both cases. Both cases had previously diagnosted on a first level hospital, but the shortage on public hospitals’ budgets is a negative situation for these couple of patients from Pillaro that need finantial sponsorship to go above from the hart desease limitations.

A 60% of the medical consults, between the age group of 6 up to 15, Intestinal Parasitosis was often to be found, whereas a 42% presented dermic problems specially owed by Micosis (phungus), and also “Dermic Irritation at the Touch”. Another diagnosts, with not to much epidemiologic relevance were found and treated righ away that moment. Contrary of what we expected, “the Brochy-Pulmonary deseases” weren’t really often to find on the inhabitants of Pillaro. Therefore, we have realized that our idea of finding Respiratory deseases based upon the idea that housing and living conditions would be a relevant reason to predict so….. was not more than  a wrong prediction.
The presence of a Dermathologist helped taking care of at least 47% of our total patients. They all were part of the Age Group between 16 up to 45 year olds. Most of them were treated because of skin deseases, such as acne, hypercromies, and “Pink Skyn” desease. There were 28 consults owed to Dermathologic reasons. The related Bone-Muscular issues like Low Back problem or Reumatic Arthritis were easy to find on a 39,6 %, along to stomach limitations such as Acid Reflex desease, Licking Ulceras, etc on a 38% out of our total patients. The possible explanation to these stomach issues might be because of the medication these patients would have to use daily. The Urinary Infections were also found on a 18% out of our total patients. We aslo the Menopausic Sindrom with a 15,8% out of our total patients. It was neat to see the absence of pregnant women under medical consult. Arterial Hypertension was found on a 13% with the modality of on the majority of our cases no previous treatments were diagnosted for our patients in any of their cases. This was because most of our patients didn’t have an idea of how to control Hypertension, and in some cases didn’t even know what that was all about. It was a great think to explain them the envolved risks with hypertension, and to provide medices for them.
The patients between 46 and 60 years presented a high percentage of Bone-Muscular limitations, especially Low Back Pain on a 47,6 % . Most of these patients were farmers, which forced them to lift daily heavy stuff and do hard work. We noticed that the lack of good back positioning during any every- day life in addition to the daily hard work they do had a clear influence for Low Back desease. The High Blood desease ( Hypertension) has a 23,7% from our consults, which makes it a high incidence desease, along to Gastric Illnesses that seem to be related with the food the locals from Pillaro eat. Beside,  alcohol consume is very popular within these people. It was detected 3 cases of alcoholism, only in men though.
Accoriding to what was previously mentioned, the Eldery people group did consult our physicians on a high percentage of 29,1% out of the total number of patients seen. The Doctor encharge to visit the local Eldery Home found out that this facility was totally cold and lacked of recreational areas as well as an appropiate area where these old patients would get stimulated. Aparently they seem to be abandoned by their direct relatives, hoping that the care offered by this Eldery House would be good enough for them. Unfortunatly, this Eldery House had few workers that sometimes weren’t quite ideal for such duties there.
The Plastic Surgeon did 24 procedures, basically “Exeresis of Nevus” (malformation-extractions from any part of the body).It was also done a “Zeta-Plastia”, which was a re-construction using a retroarticular skin implant. All the patients seen were given by our pharmacy their medical prescription, with an average of at least 2 medicines given to each patient. Some of the Dental cases needed medicine too. A total of 327 prescriptions were given by our pharmacy. On the Dental department, 55 patients were seen and given 70 Fillings, 17 Cleannings, and 17 Pullings done (Exodontic procedures). We also counted with the presece of our Dental Lab Technitian who finished 9 Partial Dentures. Mostly owed to the lack of time, 4 surguries were rescheduled for this upcomming Friday 8th of June, 2007 at the Foundation facilities’ ubicated at the Abascal and Portete streets. Somewhere between 4 pm. that Saturday, June 2th we finished with our mission and happy to know that we added somewho to the welfare of our fellow brothern there in Pillaro. The moral commetment that we have towards the Charity Anywhere cause still is alive within us, only waiting for the next chance to Be Good and Do Good once again.
